I have contacted a business that will fund art however they need information (forms and contact information) that will make both sides known to each other. Does anyone have a broker agreement that I can use that protects me - and compensates me - as the facilitator of the deal?

You need a broker agreement with the funding company. No need to overcomplicate this.
