a. Breakdown of wages by type and quarter (CONFIRM PPP Calculation is Subtracted from the ERC)

8) ERC Qualification (Reason) from ERC Filer

9) 2 Most RECENT Quarters of 941's

10) 1- Current Year of Business Tax Return

11) W9

a. By the Authorized Signer(s)

12) Copy of the Operating Agreement, By-laws, or Corporate Formation
