I specialize in RE deals. Medical offices are very doable. Rates typically in the single digits, 30 year term.
The challenge is "$400k in equity" as it does not state what the available equity for funding as Jerry noted above. What matters is the value of the property (what it would appraise for) and the ultimate LTV to determine cash-out options.
If it is a full doc deal, sometimes I can get as high as 80% LTV.
